This week QualityTools spotlights tools related to congestive heart failure (CHF), or heart failure. CHF is a condition in which the heart can't pump enough blood to the body's other organs. It is very common and can be a serious, long-term condition. According to the American Heart Association, more than 5 million Americans are living with heart failure, and 550,000 new cases are diagnosed each year. See the newly published tools from the Boston Medical Center and the Society of Hospital Medicine that can help providers manage patients with heart failure.
QualityTools is moving to the AHRQ Health Care Innovations Exchange (http://www.innovations.ahrq.gov), a new program designed to support health care professionals in identifying, sharing, and adopting innovations and tools that improve health care quality.
This is the last publication cycle for this site. QualityTools will remain available at this location until April. During this time, no additional tools will be posted, but the present tool collection will still be available.
In early April, AHRQ will launch the Innovations Exchange Web site with a new searchable database of innovations and tools. The database will include the complete QualityTools collection as well as new tools, which we will continue to add.
